Roles & Responsibilities

• Provide compassionate hands-on care and assistance with activities of daily living to support resident comfort and dignity
• Take and record vital signs accurately and promptly to support clinical decision making
• Assist with safe transfers, mobility, and positioning while following facility safety protocols
• Document resident status, care provided, and any changes promptly and clearly in the medical record
• Collaborate with nurses and interdisciplinary team members to ensure coordinated, person-centered care
• Adhere to infection control practices and maintain a clean, safe environment for residents
• Support residents’ emotional needs by offering respectful communication and companionship
• Respond to resident needs and call lights in a timely, professional manner

Qualifications

• Current Licensed Nursing Assistant (LNA) credential (NH or VT as applicable)
• T-LNA certification is a plus
• Current CPR/BLS certification (or willing to obtain)
• Comfortable working weekends and some holidays as part of the rotation
• Strong communication skills and a team-first attitude
• Previous experience in long-term care or skilled nursing preferred

Education

• High school diploma or GED and completion of an approved LNA training program

Knowledge & Skills

• Practical knowledge of ADLs (bathing, dressing, toileting, feeding)
• Ability to take and document vital signs accurately
• Familiarity with basic infection control and safety procedures
• Good observational skills to notice and report changes in resident condition
• Reliable documentation and charting skills
• Effective, compassionate communication with residents and family members
