Roles & Responsibilities

• Provide attentive 1:1 patient care to a current resident as assigned
• Assist residents with activities of daily living including bathing, dressing, grooming and mobility
• Observe and report changes in resident condition to nursing staff promptly
• Follow individualized care plans and support implementation of therapy goals
• Maintain clear, accurate documentation of care and resident responses
• Promote a safe, respectful and dignified environment for residents
• Collaborate with nursing and interdisciplinary team members to meet resident needs

Qualifications

• Current Certified Nursing Assistant (CNA) credential
• Comfortable and experienced providing 1:1 patient care
• Ability to work a PRN block schedule as required by the facility
• Compassionate approach and strong communication skills with residents and team

Knowledge & Skills

• Resident-centered care and one-on-one supervision
• Assistance with ADLs and mobility support
• Basic clinical observation and clear reporting skills
• Good documentation practices and care-plan follow-through
• Team collaboration and resident safety awareness
