Roles & Responsibilities

• Assess patients’ functional status and create individualized treatment plans
• Provide hands-on therapy and therapeutic exercise to progress mobility and independence
• Monitor patient progress and adjust interventions to achieve measurable outcomes
• Collaborate with interdisciplinary team members to coordinate comprehensive care
• Maintain accurate, timely documentation in the facility’s clinical record
• Educate patients and families on home programs, safety, and long-term recovery strategies

Qualifications

• Active New Jersey Physical Therapist license or ability to obtain one quickly
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Previous experience in outpatient or contract rehab settings preferred
• Strong clinical reasoning, patient communication, and outcome-focused approach
• Ability to pass facility onboarding and any required background checks

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in a related field considered

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, and manual therapy techniques
• Comfortable with documentation and electronic medical record (EMR) systems
• Good time management, prioritization, and caseload organization
• Excellent verbal and written communication with patients and teammates
• Familiarity with discharge planning and community resource coordination
