Roles & Responsibilities

• Provide high-quality physical therapy interventions and hands-on care for short- and long-term rehab residents
• Document therapy treatment and progress using Nethealth and PCC to ensure continuity and compliance
• Collaborate with nursing and interdisciplinary teams to create meaningful care plans
• Meet productivity expectations and maintain clinical efficiency (90% productivity requirement)
• Adhere to Continuum Rehabilitation Group policies and facility procedures while promoting patient safety
• Maintain professional appearance and follow dress code (black scrubs) while representing the rehab team

Qualifications

• 1-3 years of PTA experience (as listed: experience level 1-3)
• Active PTA credential or eligibility per state requirements
• Comfortable working in a skilled nursing facility (SNF) environment
• Experience with EMR/rehab documentation systems preferred (Nethealth, PCC)
• Ability to be onsite for scheduled shifts to receive guaranteed hours
