Roles & Responsibilities
- Provide compassionate direct patient care, including monitoring and documenting vital signs and changes in condition
- Administer medications and treatments accurately and safely according to facility policy
- Collaborate with the care team to create and update individualized care plans
- Perform wound care and other skilled nursing tasks within scope of practice
- Maintain clear, timely, and accurate clinical documentation in the patient record
- Communicate effectively with patients, families and interdisciplinary team members to deliver excellent care
- Respond promptly to patient needs and emergencies while maintaining calm and professional care
Qualifications
- Active Licensed Practical Nurse (LPN) license to practice in West Virginia
- Current BLS/CPR certification required
- Graduate of an accredited practical nursing program
- Comfortable working overnight shifts and providing hands-on patient care
- Preferred experience in long-term care, skilled nursing, or similar settings
Education
- Diploma or certificate from an accredited Practical Nursing program
- LPN/LVN licensure required
Knowledge & Skills
- Strong medication administration and documentation skills
- Ability to perform thorough patient assessments and report changes
- Familiarity with infection control best practices and safe patient handling
- Clear, compassionate communication and teamwork skills
- Basic charting and electronic medical record navigation
