Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day shift, 5x8) with weekend rotation (every other weekend) to meet department needs
Hours per Week: 40
Candidate Pay Rate: $67 per hour

Roles & Responsibilities
- Coordinate patient care from admission to discharge, ensuring safe transitions and continuity of care
- Lead discharge planning efforts, arranging DME, SNF placement, and ambulance transport independently
- Perform utilization review using InterQual criteria to support appropriate level-of-care decisions
- Document thoroughly and accurately in the charting system (Epic) to maintain clinical and regulatory compliance
- Collaborate with interdisciplinary teams (social services, transportation, clinicians) to develop and execute individualized plans of care
- Facilitate family meetings and contribute to treatment plan development with clarity and compassion
- Apply DRG and CMS knowledge to support regulatory and reimbursement-related processes
- Manage complex cases involving substance use, homelessness, and mental health crises with clinical judgment and resourcefulness
Qualifications
- California state clinical license required (pending accepted but must be licensed by start date)
- Minimum 2 years of acute care hospital case management experience performing care coordination and discharge planning (hospital experience is a must)
- Travel/contract experience required (first-time travelers accepted — clinical will screen)
- Comfortable working with charting systems (Epic required)
- Able to work weekend rotations and meet department scheduling needs
- Strong documentation, organizational, and communication skills
Knowledge & Skills
- Expertise in care coordination, discharge planning, and case management in an acute hospital setting
- Proficiency with Epic and computerized documentation systems
- Familiarity with InterQual criteria, DRG processes, and CMS regulations
- Experience arranging DME, SNF placements, and ambulance transport independently
- Knowledge of HMOs, IPAs, and payer considerations
- Competence managing patients with substance use, homelessness, and mental health crises
- Proficient in Microsoft Office applications
- Ability to participate in and lead family meetings and interdisciplinary planning
