Roles & Responsibilities

• Coordinate patient care from admission to discharge, ensuring safe transitions and continuity of care
• Lead discharge planning efforts, arranging DME, SNF placement, and ambulance transport independently
• Perform utilization review using InterQual criteria to support appropriate level-of-care decisions
• Document thoroughly and accurately in the charting system (Epic) to maintain clinical and regulatory compliance
• Collaborate with interdisciplinary teams (social services, transportation, clinicians) to develop and execute individualized plans of care
• Facilitate family meetings and contribute to treatment plan development with clarity and compassion
• Apply DRG and CMS knowledge to support regulatory and reimbursement-related processes
• Manage complex cases involving substance use, homelessness, and mental health crises with clinical judgment and resourcefulness

Qualifications

• California state clinical license required (pending accepted but must be licensed by start date)
• Minimum 2 years of acute care hospital case management experience performing care coordination and discharge planning (hospital experience is a must)
• Travel/contract experience required (first-time travelers accepted — clinical will screen)
• Comfortable working with charting systems (Epic required)
• Able to work weekend rotations and meet department scheduling needs
• Strong documentation, organizational, and communication skills

Knowledge & Skills

• Expertise in care coordination, discharge planning, and case management in an acute hospital setting
• Proficiency with Epic and computerized documentation systems
• Familiarity with InterQual criteria, DRG processes, and CMS regulations
• Experience arranging DME, SNF placements, and ambulance transport independently
• Knowledge of HMOs, IPAs, and payer considerations
• Competence managing patients with substance use, homelessness, and mental health crises
• Proficient in Microsoft Office applications
• Ability to participate in and lead family meetings and interdisciplinary planning
