Roles & Responsibilities
- Provide skilled physical therapy evaluations and goal-driven treatment plans
- Deliver hands-on and evidence-based interventions to improve patient function
- Collaborate with interdisciplinary team members to coordinate patient care
- Document progress and outcomes thoroughly and in a timely manner
- Educate patients and families to support safe carryover and independence
- Contribute to quality improvement initiatives and positive patient experiences
Qualifications
- Active or eligible Delaware Physical Therapist license (or ability to obtain)
- Current CPR/BLS certification
- Clinical experience providing PT care in outpatient/rehab settings preferred
- Strong communication skills and ability to work effectively across disciplines
- Comfortable with documentation in electronic medical records
Education
- Doctor of Physical Therapy (DPT) preferred; Master’s or entry-level PT credentials accepted if state-compliant
Knowledge & Skills
- Skilled in therapeutic exercise, manual therapy, gait training and mobility retraining
- Ability to assess functional deficits and set measurable goals
- Knowledge of safety, fall prevention and adaptive equipment recommendations
- Proficiency with clinical documentation and basic technology
- Patient-centered approach with strong teaching and motivational skills
