Roles & Responsibilities
- Assess patient function and mobility to create personalized treatment plans
- Develop and implement evidence-based therapeutic interventions to restore movement
- Monitor progress, adjust plans, and document outcomes clearly in the medical record
- Collaborate with nurses, physicians, and rehab team members to optimize patient care
- Educate patients and families on home exercise programs and injury prevention
- Supervise and mentor therapy assistants and support staff when applicable
- Participate in discharge planning to ensure safe transitions and continuity of care
- Maintain compliance with clinical protocols, safety standards, and documentation requirements
Qualifications
- Current, active Physical Therapist license in New Hampshire (or compact/state authorization as applicable)
- Degree in Physical Therapy (DPT, MPT, or equivalent) — see education section
- Basic Life Support (BLS) / CPR certification required; additional certifications a plus
- Comfortable working day shifts and able to cover scheduled weekends as needed
- Strong clinical reasoning, hands-on skills, and effective communication with patients and team members
- Prior experience in inpatient rehabilitation or contract assignments is preferred but not required
Education
- Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy considered
Knowledge & Skills
- Skilled in therapeutic exercise, manual therapy, and mobility training
- Proficient with patient assessment, goal setting, and functional outcome measures
- Familiarity with documentation systems and compliant charting practices
- Knowledge of fall prevention, safe transfer techniques, and assistive devices
- Strong interpersonal skills for patient education and team collaboration
- Ability to manage a diverse caseload in a fast-paced rehab environment
