Shift Type: 5 days/week (typical Monday-Friday; may include some weekends as needed), 8:00 AM - 4:30 PM Day (5x8; facility cannot accommodate 4x10s or 3x12s)

Candidate Pay Rate: $69 per hour

Roles & Responsibilities
- Perform thorough admission/start-of-care and routine visits, using OASIS guidelines to capture accurate patient status
- Provide compassionate wound care and integumentary assessments, managing dressings and treatment plans
- Administer and manage medications safely (IM, IV infusions, subcutaneous) and monitor responses
- Start and maintain IVs and perform phlebotomy with solid technique and infection-control practices
- Document care clearly and efficiently in EPIC — accurate charting is critical for continuity of care
- Assess and manage pain using appropriate scales (verbal and nonverbal) and communicate needs with the care team
- Coordinate care with PT, social services and interpreters to keep patients safe and progressing at home
- Educate patients and caregivers on care plans, medication management and infection prevention
Qualifications
- Active RN license in Oregon or ability to obtain state licensure (pending licenses accepted)
- Minimum 1 year of skilled Home Health nursing experience required
- OASIS experience required — comfortable with start-of-care, recertification, resumption and discharge processes
- BCLS (BLS) certification required
- Demonstrated EPIC/charting experience
- IV therapy and phlebotomy skills (starting/maintaining IVs) required
- Reliable transportation and willingness to drive a service area that can occasionally extend to ~50 miles
- Facility requires primary COVID vaccination series plus one booster
Knowledge & Skills
- Strong knowledge of Home Health documentation standards (OASIS)
- Proven wound care and integumentary assessment skills
- Medication assessment and safe administration (oral, IM, IV, subcutaneous)
- Pain assessment and management using appropriate scales
- Infection control precautions and best practices in the home setting
- Comfortable navigating EPIC for accurate and timely charting
- Ability to prioritize patient safety, discharge and recertification decision-making
- Skilled communicator with patients, families and interdisciplinary team members
