Roles & Responsibilities
- Provide compassionate direct care to residents, assisting with activities of daily living
- Monitor and report changes in resident condition to the care team promptly
- Perform accurate, timely documentation in the facility charting system
- Support safe and dignified transfer and mobility assistance for residents
- Follow infection control and safety protocols to protect residents and staff
- Collaborate with nurses and therapy staff to deliver coordinated care plans
- Participate in shift handoffs to ensure continuity and quality of care
Qualifications
- Active LNA (Licensed Nursing Assistant) credential required
- Eligible to work in VT (and NH if requested) for this VT/NH assignment
- Minimum experience: facility experience preferred (specific years not listed)
- Must have reliable personal transportation
- Ability to commit to every-other-weekend coverage as noted
- Current basic life support (BLS) certification if required by facility (confirm during onboarding)
Education
- Completion of a state-approved LNA/CNA training program (or equivalent)
Knowledge & Skills
- Strong resident-focused care and dignity-preserving approach
- Skilled in ADLs, transfers, and safe patient handling
- Clear, compassionate communication with residents, families, and teammates
- Accurate charting and documentation skills
- Familiarity with infection control and long-term care standards
- Teamwork and flexibility in a dynamic care environment
