Roles & Responsibilities

• Provide compassionate direct care to residents, assisting with activities of daily living
• Monitor and report changes in resident condition to the care team promptly
• Perform accurate, timely documentation in the facility charting system
• Support safe and dignified transfer and mobility assistance for residents
• Follow infection control and safety protocols to protect residents and staff
• Collaborate with nurses and therapy staff to deliver coordinated care plans
• Participate in shift handoffs to ensure continuity and quality of care

Qualifications

• Active LNA (Licensed Nursing Assistant) credential required
• Eligible to work in VT (and NH if requested) for this VT/NH assignment
• Minimum experience: facility experience preferred (specific years not listed)
• Must have reliable personal transportation
• Ability to commit to every-other-weekend coverage as noted
• Current basic life support (BLS) certification if required by facility (confirm during onboarding)

Education

• Completion of a state-approved LNA/CNA training program (or equivalent)

• Strong resident-focused care and dignity-preserving approach
• Skilled in ADLs, transfers, and safe patient handling
• Clear, compassionate communication with residents, families, and teammates
• Accurate charting and documentation skills
• Familiarity with infection control and long-term care standards
