Roles & Responsibilities

• Assess patients’ needs and create individualized case management plans
• Coordinate effective discharge planning, including SNF placement and DME orders
• Manage utilization review activities and complete prior authorizations
• Independently arrange ambulance transport and other patient logistics
• Document thoroughly and accurately in Epic and other computerized charting systems
• Facilitate and participate in family meetings to align care goals
• Collaborate with interdisciplinary teams (PT, social services, physicians) to drive safe, timely discharges
• Ensure documentation and practice meet CMS, DRG, The Joint Commission and departmental standards

Qualifications

• California state license required (pending license accepted, but must be licensed by start date)
• Minimum 2 years acute care case management/discharge planning experience in a hospital setting
• Travel experience required — seasoned travelers only (no first-time travelers)
• Weekend availability: every other weekend as part of department coverage
• Experience working with patient ratios similar to 1:25 (days) and 1:45 (weekends)
• Proven ability to arrange DME, SNF placement and ambulance transport independently
• Proficiency with Epic and computerized charting systems

Knowledge & Skills

• Epic electronic health record proficiency
• Discharge planning and care coordination
• Utilization review and prior authorization processes
• Patient assessment and case load management
• DME coordination, SNF placement and transport logistics
• Familiarity with HMOs/IPAs and payer processes
• Regulatory knowledge: CMS, DRG, The Joint Commission/core measures
• Strong interdisciplinary communication and family meeting facilitation
