Hours per Week: 38
Candidate Pay Rate: $32 per hour

Roles & Responsibilities
- Evaluate patient function using formal and informal assessments to create a clear picture of needs.
- Develop individualized, goal-driven treatment plans and update the plan of care as patients progress.
- Deliver evidence-based treatment interventions to restore or enhance daily functioning.
- Educate patients, families and caregivers on conditions, prognosis and home programs to support lasting gains.
- Supervise Occupational Therapist Assistants, home health aides and students with clear guidance and mentorship.
- Document patient response and outcomes thoroughly and in compliance with affiliate policies and regulations.
- Recommend and coordinate adaptive/supportive equipment to improve patient independence.
- Serve as case manager for assigned patients, coordinating care across the interdisciplinary team.
- Participate in team conferences, promote program development, and champion process improvement initiatives.
Qualifications
- OTR — Occupational Therapist Registered (required)
- NBCOT certification (required)
- BLS (Basic Life Support) — to be completed within one month of hire if not current
- Bachelor's degree (required); degree in occupational therapy or related field preferred
- More than 1 year of clinical experience preferred — new graduates are welcome to apply
- Ability to work independently, manage time well, and demonstrate strong problem-solving skills
- Ability to use personal protective equipment and work where exposure to communicable diseases may occur
- Mandatory OSHA training completed annually per Exposure Control Plan
Education
- Bachelor's Degree (required)
Knowledge & Skills
- Skilled in assessment, treatment planning, intervention and evaluation
- Strong documentation practices and familiarity with billing-related documentation
- Patient and family education techniques to promote home program adherence
- Experience supervising OT assistants and students
- Case management and care coordination abilities
- Comfortable with Microsoft Windows and technology used in clinical documentation
- Knowledge of home health processes and community-based care (preferred)
- Familiarity with OSHA requirements and safe handling of bloodborne pathogens
