Roles & Responsibilities

• Provide compassionate, direct patient care consistent with LPN scope of practice
• Administer medications and treatments accurately and document thoroughly
• Monitor patient status, record vital signs, and report changes to the care team
• Collaborate with RNs, physicians and support staff to implement individualized care plans
• Maintain organized, timely clinical documentation and handoffs
• Support patient and family education to promote recovery and wellness
• Respond quickly and professionally to urgent situations and changes in condition

Qualifications

• Current Licensed Practical Nurse (LPN) credential or equivalent
• Ability to work night shifts on Monday, Tuesday and Thursday for the duration of the assignment
• Clinical experience in a skilled nursing or long-term care environment is a plus
• Clear background check and required health screenings as per facility policy

Education

• Graduation from an accredited Practical Nursing program

Knowledge & Skills

• Solid fundamentals in medication administration and patient monitoring
• Strong documentation skills and familiarity with electronic charting
• Comfortable performing routine assessments and following care plans
• Effective communication and teamwork skills with multidisciplinary staff
• Reliable time management and prioritization during busy shifts
