Roles & Responsibilities
- Administer medications and perform med pass with accuracy and compassion
- Provide IV therapy and manage central/peripheral lines safely
- Perform tracheostomy care and PEG tube management for complex rehab patients
- Deliver wound care and pressure injury prevention using best practices
- Assist with mobility and transfers using Hoyer lifts and safe patient handling techniques
- Support peritoneal dialysis (PD) needs and collaborate with specialty teams
- Document thoroughly and efficiently in the EMR to ensure continuity of care
- Collaborate with the interdisciplinary rehab team to create meaningful care plans
- Coach and mentor techs and support staff to maintain high-quality patient care
Qualifications
- Active Registered Nurse license (eligible to practice in Virginia or via compact privilege)
- Minimum 2 years of recent nursing experience; SNF background required
- Experience in rehab or med-surgical nursing with familiarity in telemetry a plus
- Proficiency with EMR documentation and electronic charting
- Comfort and competence with Hoyer lifts, trach care, PEG tube care, wound care, IV therapy, and PD
- Strong teamwork, communication skills, and a positive, patient-centered attitude
- Current BLS certification; other certifications encouraged as applicable
Education
- Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred
Knowledge & Skills
- Med pass and basic med-surgical nursing
- IV therapy and vascular access management
- Trach care and respiratory support knowledge
- Wound assessment and dressing techniques
- Hoyer lift operation and safe patient handling
- PEG tube care and enteral nutrition basics
- Peritoneal dialysis familiarity
- EMR proficiency and accurate clinical documentation
- Telemetry monitoring and interpreting basic cardiac rhythms
