Roles & Responsibilities
- Evaluate patients comprehensively and develop individualized, evidence-based treatment plans
- Deliver hands-on therapy and therapeutic exercise to promote mobility, function, and independence
- Document patient progress clearly and timely in the medical record to support continuity of care
- Collaborate with nursing, rehab aides, case managers, and family members to coordinate care
- Educate patients and caregivers on safe transfers, home exercise programs, and fall prevention
- Recommend and fit adaptive equipment and durable medical equipment as needed
- Participate in discharge planning to ensure safe transitions and ongoing recovery
- Maintain compliance with facility policies, regulatory standards, and professional best practices
Qualifications
- Current, unrestricted Physical Therapist license in Massachusetts (required)
- Doctor of Physical Therapy (DPT) or equivalent clinical qualification preferred
- BLS/CPR certification required or willingness to obtain prior to start
- Experience in post-acute, rehab, or inpatient therapy settings preferred
- Strong clinical assessment, manual therapy, and therapeutic exercise skills
- Excellent communication and teamwork abilities, with patient-centered approach
Education
- Doctor of Physical Therapy (DPT) preferred; Bachelor of Science in Physical Therapy or related degree accepted
Knowledge & Skills
- Skilled in musculoskeletal and neuromuscular evaluation and treatment
- Knowledge of therapeutic modalities, gait training, and functional mobility interventions
- Proficient documentation and outcomes tracking for regulatory and clinical needs
- Ability to adapt treatment plans for varied acuity levels and complex cases
- Strong interpersonal skills for working with patients, families, and the care team
