Roles & Responsibilities

• Evaluate patients comprehensively and develop individualized, evidence-based treatment plans
• Deliver hands-on therapy and therapeutic exercise to promote mobility, function, and independence
• Document patient progress clearly and timely in the medical record to support continuity of care
• Collaborate with nursing, rehab aides, case managers, and family members to coordinate care
• Educate patients and caregivers on safe transfers, home exercise programs, and fall prevention
• Recommend and fit adaptive equipment and durable medical equipment as needed
• Participate in discharge planning to ensure safe transitions and ongoing recovery
• Maintain compliance with facility policies, regulatory standards, and professional best practices

Qualifications

• Current, unrestricted Physical Therapist license in Massachusetts (required)
• Doctor of Physical Therapy (DPT) or equivalent clinical qualification preferred
• BLS/CPR certification required or willingness to obtain prior to start
• Experience in post-acute, rehab, or inpatient therapy settings preferred
• Strong clinical assessment, manual therapy, and therapeutic exercise skills
• Excellent communication and teamwork abilities, with patient-centered approach

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor of Science in Physical Therapy or related degree accepted

Knowledge & Skills

• Skilled in musculoskeletal and neuromuscular evaluation and treatment
• Knowledge of therapeutic modalities, gait training, and functional mobility interventions
• Proficient documentation and outcomes tracking for regulatory and clinical needs
• Ability to adapt treatment plans for varied acuity levels and complex cases
• Strong interpersonal skills for working with patients, families, and the care team
