Roles & Responsibilities

• Evaluate and assess patients to create individualized treatment plans
• Provide skilled physical therapy interventions to improve mobility and function
• Document patient progress clearly and timely in the facility electronic records
• Collaborate with interdisciplinary team members to optimize patient outcomes
• Educate patients and families on home programs and injury prevention
• Participate in discharge planning and ensure safe transitions of care

Qualifications

• Valid Physical Therapist license in Indiana or eligibility to obtain one
• Degree in Physical Therapy (see education) from an accredited program
• Current BLS/CPR certification or willingness to obtain upon hire
• Comfort working in a contract/local facility setting and adapting to site policies
• Strong communication, clinical reasoning, and patient-centered skills
• Prior experience in outpatient, inpatient rehab, or similar settings is a plus

Education

• Doctor of Physical Therapy (DPT) preferred; Master of Physical Therapy considered

Knowledge & Skills

• Skilled in patient assessment, differential diagnosis, and treatment planning
• Proficient in therapeutic exercise, manual therapy, and gait training
• Comfortable with documentation standards and electronic health record systems
• Knowledge of safety, fall prevention, and discharge planning best practices
• Excellent interpersonal skills for patient and team communication
