Roles & Responsibilities
- Assist the Physical Therapist in implementing individualized treatment plans that help patients regain mobility and independence
- Provide therapeutic exercises and targeted mobility training to improve patient function
- Monitor patient progress and communicate updates and changes to the supervising PT
- Maintain accurate, timely documentation to support continuity of care
- Collaborate with the interdisciplinary care team to ensure coordinated, high-quality outcomes
- Deliver compassionate, patient-centered care that respects each person’s goals and dignity
Qualifications
- Graduate of an accredited Physical Therapist Assistant (PTA) program
- Active California PTA license (required)
- SNF or similar clinical experience preferred
- Strong communication and teamwork skills to thrive in an interdisciplinary setting
- Commitment to quality patient care and professional documentation
- Willingness to complete required credentialing and compliance steps
Education
- Graduate of an accredited Physical Therapist Assistant (PTA) program
Knowledge & Skills
- Clinical knowledge of therapeutic exercises, mobility training, and ADL support
- Ability to follow and implement PT-developed treatment plans
- Familiarity with timely and accurate clinical documentation and EHR systems
- Strong interpersonal skills for working with patients and multidisciplinary teams
- Good observational skills for monitoring progress and reporting changes
- Patient-centered approach with focus on safety and functional outcomes
