Roles & Responsibilities

• Perform thorough patient assessments and evaluations to establish individualized, goal-oriented treatment plans
• Deliver evidence-based outpatient therapy interventions and adapt care for Long-Term Care and Skilled Swing patients as needed
• Implement treatment plans, track progress, and modify interventions to maximize functional outcomes
• Educate patients and caregivers on home exercise programs, safety, and strategies to improve independence
• Collaborate closely with physicians, nurses, and ancillary staff to ensure coordinated, high-quality care
• Document patient status and progress clearly and in a timely manner
• Participate in a feedback-friendly culture — both offering and receiving suggestions to continuously improve patient care

Qualifications

• Active Physical Therapist (PT) license in Montana or ability to obtain licensure prior to start
• Graduate degree in physical therapy (Doctor of Physical Therapy or Master's in Physical Therapy) from an accredited program
• Experience treating outpatient populations; comfortable providing some Long-Term Care and Skilled Swing services
• Strong communication skills and ability to work collaboratively within a small interdisciplinary team
• Flexible, open-minded approach and a willingness to both give and receive constructive suggestions
• Reliable availability for a full-time Monday–Friday daytime schedule

Education

• Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (MPT) — graduation from an accredited program
