Roles & Responsibilities

• Evaluate patients to create individualized treatment plans that restore mobility and independence
• Deliver hands-on and evidence-based physical therapy interventions with compassion
• Document patient progress clearly and timely in the electronic medical record
• Collaborate with nurses, physicians, and rehabilitation staff to coordinate care goals
• Educate patients and families on home programs and fall-prevention strategies
• Participate in team meetings and discharge planning to ensure safe transitions

Qualifications

• Current or compact-eligible Physical Therapist license (state licensure required or ability to obtain)
• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy accepted where applicable
• Basic Life Support (BLS/CPR) certification
• Clinical experience in inpatient rehabilitation or skilled nursing is a strong plus
• Comfortable with documentation in electronic medical records and strong communication skills
• Able to work a schedule that includes weekends and provide consistent, high-quality care

Education

• Doctor of Physical Therapy (DPT) preferred
• Master’s or Bachelor’s in Physical Therapy acceptable where applicable

Knowledge & Skills

• Skilled in assessment of mobility, balance, strength, and functional performance
• Proficient with therapeutic modalities, manual therapy, and therapeutic exercise prescription
• Strong patient education and family training abilities
• Knowledgeable about discharge planning and community resources
• Team-player attitude with good time management and organizational skills
