Roles & Responsibilities
- Evaluate patients to create individualized treatment plans that restore mobility and independence
- Deliver hands-on and evidence-based physical therapy interventions with compassion
- Document patient progress clearly and timely in the electronic medical record
- Collaborate with nurses, physicians, and rehabilitation staff to coordinate care goals
- Educate patients and families on home programs and fall-prevention strategies
- Participate in team meetings and discharge planning to ensure safe transitions
Qualifications
- Current or compact-eligible Physical Therapist license (state licensure required or ability to obtain)
- Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy accepted where applicable
- Basic Life Support (BLS/CPR) certification
- Clinical experience in inpatient rehabilitation or skilled nursing is a strong plus
- Comfortable with documentation in electronic medical records and strong communication skills
- Able to work a schedule that includes weekends and provide consistent, high-quality care
Education
- Doctor of Physical Therapy (DPT) preferred
- Master’s or Bachelor’s in Physical Therapy acceptable where applicable
Knowledge & Skills
- Skilled in assessment of mobility, balance, strength, and functional performance
- Proficient with therapeutic modalities, manual therapy, and therapeutic exercise prescription
- Strong patient education and family training abilities
- Knowledgeable about discharge planning and community resources
- Team-player attitude with good time management and organizational skills
