Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day shift, 35 hours/week, No weekend rotation)
Hours per Week: 35
Candidate Pay Rate: $40 per hour

Roles & Responsibilities
- Provide patient-centered physical therapy evaluations and interventions
- Develop and implement individualized treatment plans to restore function and mobility
- Monitor and document patient progress clearly and timely
- Collaborate with nurses, physicians and rehab team members to optimize outcomes
- Educate patients and families on home programs and safe activity progression
- Participate in discharge planning to ensure continuity of care
Qualifications
- Licensed Physical Therapist in the state of Wisconsin (or eligible to obtain licensure)
- Current CPR/BLS certification or willingness to obtain upon hire
- Strong communication skills and a collaborative mindset
- Experience in inpatient rehabilitation or related settings preferred
- Ability to complete required background checks and onboarding documentation
Education
- Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy where applicable
Knowledge & Skills
- Skilled in assessment and development of therapeutic treatment plans
- Proficient with therapeutic exercise, gait training and functional mobility interventions
- Solid documentation practices and attention to clinical detail
- Ability to work effectively as part of an interdisciplinary team
- Patient education and family coaching for home exercise adherence
