Roles & Responsibilities

• Evaluate patients’ functional mobility and create individualized treatment plans
• Implement therapeutic interventions to improve strength, balance, and mobility
• Monitor patient progress and update care plans with measurable goals
• Collaborate with nurses, physicians, and rehab team members to coordinate care
• Educate patients and families on exercise programs and safe mobility techniques
• Document treatments and outcomes accurately in the patient record
• Support discharge planning and recommend appropriate follow-up services

Qualifications

• Active Physical Therapist license in California or eligible to obtain licensure
• Doctor of Physical Therapy (DPT) or equivalent degree (preferred)
• Basic Life Support (BLS) / CPR certification (or willingness to obtain)
• Prior experience in inpatient rehabilitation, subacute, or outpatient settings preferred
• Strong clinical assessment and manual therapy skills
• Excellent communication skills and a collaborative mindset

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy acceptable where applicable

Knowledge & Skills

• Skilled in musculoskeletal and neurologic rehabilitation techniques
• Proficient with therapeutic exercise prescription and mobility training
• Experience with assistive devices, gait training, and fall prevention strategies
• Comfortable documenting in electronic health records and following regulatory standards
• Ability to prioritize caseload and manage time effectively
• Patient-centered approach with strong education and discharge planning skills
