Roles & Responsibilities
- Provide skilled occupational therapy evaluations and treatments in patients' homes, focusing on meaningful ADLs and functional independence
- Complete thorough OASIS documentation and chart using EPIC Remote Client and Rover phone app
- Develop, implement and update individualized Plans of Care and conduct timely reassessments
- Deliver patient- and family-centered education to support safe home transitions and self-care
- Assess mobility, lifting/transfer needs and recommend adaptive equipment or home modifications
- Manage a caseload of up to approximately 6 patient visits per day while maintaining expected productivity standards
- Coordinate schedules and routes to optimize patient contact time and minimize windshield time
- Adhere to required certifications and facility policies, including BCLS and vaccination requirements
Qualifications
- Current Oregon occupational therapy license (required)
- BCLS (Basic Cardiac Life Support) certification (required)
- Minimum 1 year of skilled home health experience
- OASIS experience is required—please call out if you do not have it
- Comfortable using EPIC Remote Client for Home Health and Rover app for charting
- Reliable transportation (traveler expected to have own vehicle)
- Facility requires primary COVID vaccination series plus one booster
- First-time travel therapists are welcome
Education
- Occupational Therapy degree (OTR/OT) or equivalent
- Current state licensure in Oregon
Knowledge & Skills
- OASIS documentation and home health-specific charting
- EPIC Remote Client and Rover mobile charting workflow
- Assessment, evaluation, plan of care development and reassessments
- Activities of Daily Living (ADL) training and adaptive equipment recommendations
- Lifting and transfer assessment and safe patient handling
- Wheelchair evaluation & positioning and splinting (dynamic, functional, static, serial/inhibitory)
- Orthopedics: hip/knee post-op care, tendon repair, joint mobilization
- Neuro rehab skills: CVA/post-stroke, TBI, Parkinson’s, MS, dementia care
- Prosthetics/orthotics familiarity and amputee care
- Strong patient/family education, safety/injury prevention and interdisciplinary communication
- Billing/documentation best practices for home health
