Roles & Responsibilities
- Perform thorough evaluations to assess patients’ speech, language, and swallowing needs
- Deliver daily treatments tailored to each patient’s goals and progress
- Develop and manage discharge plans to ensure safe transitions and continuity of care
- Document clinical encounters and progress notes accurately and timely
- Collaborate with the interdisciplinary team to optimize patient outcomes
- Meet and maintain the therapy productivity requirement and follow CRG policies and procedures
Qualifications
- Active SLP credential/licensure as required by state regulations (ensure New Mexico eligibility)
- 1–3 years of clinical experience (prior SNF experience strongly preferred)
- Comfortable with evaluations, daily treatment plans, and discharge planning
- Ability to meet a productivity requirement of 90% and follow facility policies
- Professional presentation and adherence to dress code (black scrubs)
- Clear communication skills and the ability to work well within a multidisciplinary team
Knowledge & Skills
- Skilled in adult dysphagia and language/cognitive therapy techniques
- Experience with SNF workflows, documentation, and discharge processes
- Strong clinical reasoning and progress-driven treatment planning
- Time management to balance evaluations, treatments, and documentation
- Collaborative approach to working with nursing and rehab staff
- Familiarity with productivity tracking and meeting performance targets
- Patient-centered communication and family education
