Roles & Responsibilities
- Assess residents' communication, swallowing, and cognitive-communication needs with compassion and clinical rigor
- Develop and implement individualized treatment plans that improve function and quality of life
- Provide skilled speech-language therapy interventions in a skilled nursing environment
- Document progress accurately and timely to meet facility and regulatory standards
- Collaborate with nursing, therapy colleagues, and families to coordinate care
- Educate residents and caregivers on strategies to carryover gains outside of therapy sessions
- Meet the facility productivity expectation (90%) while delivering quality care
- Adhere to Continuum Rehabilitation Group policies and procedures and facility dress code (black scrubs)
Qualifications
- 1-3 years of experience as an SLP (as listed in the job) — new grads with strong clinical skills may inquire
- Appropriate SLP credentials and state licensure as required by the facility or state
- Comfortable working in a skilled nursing facility (SNF) practice setting
- Willingness to follow CRG policies and productivity expectations
- Ability to be present in the building to receive guaranteed hours
Knowledge & Skills
- Skilled in SLP assessment and treatment planning for adults in SNF settings
- Strong documentation and time management skills
- Excellent communication and teamwork abilities
- Knowledge of dysphagia management, cognitive-communication therapy, and restorative approaches
- Ability to meet productivity targets while maintaining patient-centered care
