Roles & Responsibilities

• Provide skilled physical therapy evaluations, create individualized treatment plans, and deliver evidence-based interventions
• Collaborate with the interdisciplinary team to optimize resident outcomes and support discharge planning
• Document thoroughly and accurately in accordance with facility standards and CRG policies and procedures
• Meet productivity and performance targets to ensure consistent, high-quality care (90% productivity requirement)
• Demonstrate professionalism on-site (dress code: black scrubs) and maintain punctual, reliable presence to receive guaranteed hours
• Communicate clearly with residents, families, and staff to educate and motivate toward functional goals

Qualifications

• PT or DPT credential listed as primary skills (PT/DPT)
• 1-3 years of clinical experience preferred (posting specifies 1-3 years)
• Experience in a skilled nursing facility (SNF) or post-acute setting is valued
• Willingness to follow Continuum Rehabilitation Group (CRG) policies & procedures
• Ability to meet productivity expectations and maintain timely, compliant documentation
• Comfort adhering to facility dress code (black scrubs) and being on-site to receive guaranteed hours

Education

• Doctor of Physical Therapy (DPT) preferred or PT credential (as applicable)

Knowledge & Skills

• Skilled nursing facility therapy practices and post-acute care workflows
• Assessment and treatment planning for geriatric and medically complex populations
• Strong documentation skills and productivity tracking
• Interdisciplinary collaboration and clear patient/family communication
• Time management, reliability, and adaptability in a clinical setting
