Roles & Responsibilities

• Perform thorough evaluations to identify patient speech, language and swallowing needs
• Provide daily treatments tailored to each resident’s goals and progress
• Manage and coordinate safe, timely discharges with the interdisciplinary team
• Document clinical care clearly and follow facility and CRG policies and procedures
• Collaborate with nurses, therapists and family members to create meaningful care plans
• Maintain required productivity standards and contribute to a positive therapy culture

Qualifications

• Licensed/credentialed SLP (role listed as SLP) with clinical experience
• 1-3 years of relevant clinical experience (per job posting)
• Prior SNF experience strongly preferred
• Ability to be present in the building to receive guaranteed hours (facility policy)
• Comfort following employer policies, productivity expectations (90%), and dress code (black scrubs)

Knowledge & Skills

• Skilled in evaluations, daily treatments and patient discharges
• Strong clinical documentation and time-management abilities
• Ability to work collaboratively within a therapy team
• Familiarity with SNF workflows and skilled nursing patient population
• Comfort meeting productivity expectations while delivering high-quality care
