Roles & Responsibilities
- Perform thorough evaluations to identify patient speech, language and swallowing needs
- Provide daily treatments tailored to each resident’s goals and progress
- Manage and coordinate safe, timely discharges with the interdisciplinary team
- Document clinical care clearly and follow facility and CRG policies and procedures
- Collaborate with nurses, therapists and family members to create meaningful care plans
- Maintain required productivity standards and contribute to a positive therapy culture
Qualifications
- Licensed/credentialed SLP (role listed as SLP) with clinical experience
- 1-3 years of relevant clinical experience (per job posting)
- Prior SNF experience strongly preferred
- Ability to be present in the building to receive guaranteed hours (facility policy)
- Comfort following employer policies, productivity expectations (90%), and dress code (black scrubs)
Knowledge & Skills
- Skilled in evaluations, daily treatments and patient discharges
- Strong clinical documentation and time-management abilities
- Ability to work collaboratively within a therapy team
- Familiarity with SNF workflows and skilled nursing patient population
- Comfort meeting productivity expectations while delivering high-quality care
