Roles & Responsibilities
- Provide skilled speech-language evaluations and develop individualized treatment plans for SNF residents
- Deliver evidence-based therapy for communication, swallowing, and cognitive-communication disorders
- Collaborate closely with PT, OT, nursing and family members to coordinate care and maximize outcomes
- Document timely, clear progress notes and treatment plans that meet facility and regulatory standards
- Support facility productivity expectations and follow Continuum Rehabilitation Group policies and procedures
- Participate in team meetings and contribute to a positive, resident-centered therapy culture
Qualifications
- Active SLP license or eligibility to obtain licensure in New Mexico
- 1-3 years clinical experience (SNF/long-term care experience is a strong match)
- Familiarity with skilled nursing facility workflows and documentation requirements
- Strong interpersonal and communication skills to work with residents, families, and interdisciplinary teams
- Ability to follow facility dress code and on-site presence requirements
Knowledge & Skills
- Expertise in adult dysphagia assessment and management
- Skilled in cognitive-communication and language therapy for medically complex patients
- Proficient documentation and regulatory compliance knowledge
- Ability to prioritize caseload and meet productivity expectations
- Comfortable collaborating within an interdisciplinary rehab team
