Roles & Responsibilities

• Provide skilled speech-language evaluations and develop individualized treatment plans for SNF residents
• Deliver evidence-based therapy for communication, swallowing, and cognitive-communication disorders
• Collaborate closely with PT, OT, nursing and family members to coordinate care and maximize outcomes
• Document timely, clear progress notes and treatment plans that meet facility and regulatory standards
• Support facility productivity expectations and follow Continuum Rehabilitation Group policies and procedures
• Participate in team meetings and contribute to a positive, resident-centered therapy culture

Qualifications

• Active SLP license or eligibility to obtain licensure in New Mexico
• 1-3 years clinical experience (SNF/long-term care experience is a strong match)
• Familiarity with skilled nursing facility workflows and documentation requirements
• Strong interpersonal and communication skills to work with residents, families, and interdisciplinary teams
• Ability to follow facility dress code and on-site presence requirements

Knowledge & Skills

• Expertise in adult dysphagia assessment and management
• Skilled in cognitive-communication and language therapy for medically complex patients
• Proficient documentation and regulatory compliance knowledge
• Ability to prioritize caseload and meet productivity expectations
• Comfortable collaborating within an interdisciplinary rehab team
