Roles & Responsibilities

• Evaluate patients and design meaningful, functional occupational therapy plans that support independence
• Deliver skilled OT interventions in a skilled nursing facility setting to improve patient outcomes
• Document progress and outcomes accurately and timely following facility and CRG policies
• Collaborate with therapy team and interdisciplinary staff to create cohesive, person-centered care plans
• Educate patients, families, and staff on therapeutic strategies and safe functional techniques
• Meet productivity expectations (90% productivity requirement) while maintaining clinical quality

Qualifications

• 1-3 years of occupational therapy experience (per job posting)
• Current Occupational Therapist licensure in New Mexico or eligibility to obtain licensure
• Ability to be on-site (therapist must be present in building to receive guaranteed hours)
• Comfort following company policies and productivity standards (CRG P & P; 90% productivity target)
• Professional dress in black scrubs as specified by the facility

Knowledge & Skills

• Skilled Nursing Facility (SNF) experience and understanding of post-acute care needs
• Strong evaluation and treatment planning skills for functional independence
• Thorough documentation skills and familiarity with SNF workflows
• Ability to collaborate with interdisciplinary teams and communicate effectively
• Time management and productivity focus in a fast-paced therapy environment
