Roles & Responsibilities

• Assess residents' conditions and create thoughtful, person-centered care plans
• Implement nursing interventions and provide direct care with attention to detail
• Administer medications safely and accurately, maintaining high standards
• Supervise CNAs and support staff to ensure continuity and quality of care
• Document care and changes thoroughly in the Electronic Medical Records (EMR)
• Communicate professionally and compassionately with residents, families, and the care team
• Participate in care coordination to promote positive resident outcomes

Qualifications

• Active California LVN license (required)
• Skilled nursing experience with long-term care residents (required)
• Proven ability to pass nursing medical competency testing
• Current BLS certification (required)
• Comfortable using Electronic Medical Records (EMR) systems
• Strong English verbal and written communication skills
• Basic computer skills and experience supervising CNAs

Education

• Completion of an accredited Vocational Nursing (LVN) program or equivalent

Knowledge & Skills

• Nursing assessment and individualized care planning
• Medication administration and safe documentation practices
• Hands-on experience in skilled nursing and long-term care settings
• Proficiency with Electronic Medical Records (EMR) workflows
• BLS and basic life support protocols
• Interpersonal skills for respectful patient and team interactions
• Ability to supervise and mentor nursing assistants
