Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day), 35 hours/week, no weekend rotation

Candidate Pay Rate: $46 per hour

Roles & Responsibilities
- Evaluate and assess residents to create individualized, goal-driven treatment plans
- Deliver skilled therapeutic interventions to improve mobility, balance, and function
- Collaborate with nursing and rehabilitation team members to coordinate care
- Document progress clearly and timely in the facility EMR to support continuity of care
- Train and educate residents and families on safe mobility, fall prevention, and home exercise programs
- Supervise and mentor therapy assistants and support staff to amplify patient outcomes
Qualifications
- Licensed Physical Therapist in Washington state (required)
- Current BLS/CPR certification or willingness to obtain upon hire
- Experience working in a Skilled Nursing Facility (SNF) or geriatrics preferred
- Strong clinical reasoning and documentation skills
- Excellent communication and teamwork mindset with a patient-centered approach
- Ability to pass background checks and meet facility onboarding requirements
Education
- Doctor of Physical Therapy (DPT) preferred, or equivalent degree as required for state licensure
- Completion of accredited physical therapy program required for licensure
Knowledge & Skills
- Geriatric rehabilitation and skilled nursing care principles
- Mobility, transfers, gait training, and fall prevention strategies
- Therapeutic exercise, manual therapy techniques, and ADL retraining
- Familiarity with documentation best practices and electronic medical records
- Infection control, safety protocols, and interdisciplinary care coordination
- Time management and caseload prioritization in an SNF environment
