Roles & Responsibilities
- Assess patients' mobility, strength, balance and functional needs to create personalized care plans
- Develop and implement therapeutic interventions including therapeutic exercise and manual techniques
- Document progress and outcomes accurately in the medical record for continuity of care
- Collaborate with interdisciplinary teams to coordinate patient-centered treatment
- Educate patients and caregivers on home exercise programs and injury prevention
- Monitor and adjust treatment plans based on patient response and goals
- Promote a culture of safety, dignity and rehabilitation-focused care
Qualifications
- Current, unrestricted Physical Therapist license in Tennessee or eligibility to obtain licensure
- Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in Physical Therapy considered
- BLS (Basic Life Support) certification required or able to obtain
- Comfortable with outpatient and rehabilitative care in a long-term/assisted living setting
- Strong clinical reasoning, documentation skills, and a patient-first attitude
- Excellent communication and teamwork skills; friendly bedside manner
- Previous clinical experience preferred but new graduates with strong internships encouraged to apply
Education
- Doctor of Physical Therapy (DPT) preferred
- Bachelor's or Master's in Physical Therapy acceptable with relevant clinical experience
Knowledge & Skills
- Skilled in patient assessment, therapeutic exercise and manual therapy
- Knowledge of gait training, balance retraining and functional mobility interventions
- Familiarity with common outcome measures and goal-driven treatment planning
- Proficient documentation skills and comfort with electronic health records
- Ability to educate patients and families in clear, supportive language
- Strong time management and ability to prioritize caseload effectively
