Roles & Responsibilities

• Assess patients' mobility, strength, balance and functional needs to create personalized care plans
• Develop and implement therapeutic interventions including therapeutic exercise and manual techniques
• Document progress and outcomes accurately in the medical record for continuity of care
• Collaborate with interdisciplinary teams to coordinate patient-centered treatment
• Educate patients and caregivers on home exercise programs and injury prevention
• Monitor and adjust treatment plans based on patient response and goals
• Promote a culture of safety, dignity and rehabilitation-focused care

Qualifications

• Current, unrestricted Physical Therapist license in Tennessee or eligibility to obtain licensure
• Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in Physical Therapy considered
• BLS (Basic Life Support) certification required or able to obtain
• Comfortable with outpatient and rehabilitative care in a long-term/assisted living setting
• Strong clinical reasoning, documentation skills, and a patient-first attitude
• Excellent communication and teamwork skills; friendly bedside manner
• Previous clinical experience preferred but new graduates with strong internships encouraged to apply

Education

• Doctor of Physical Therapy (DPT) preferred
• Bachelor's or Master's in Physical Therapy acceptable with relevant clinical experience

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise and manual therapy
• Knowledge of gait training, balance retraining and functional mobility interventions
• Familiarity with common outcome measures and goal-driven treatment planning
• Proficient documentation skills and comfort with electronic health records
• Ability to educate patients and families in clear, supportive language
• Strong time management and ability to prioritize caseload effectively
