Roles & Responsibilities

• Deliver compassionate, hands-on nursing care to residents to improve daily living and comfort
• Administer medications and treatments accurately and document interventions clearly
• Perform timely assessments and communicate changes in patient condition to the care team
• Collaborate with interdisciplinary team members to create and follow individualized care plans
• Support resident mobility, personal hygiene and nutrition with dignity and respect
• Maintain accurate electronic health records and complete required documentation on time
• Participate in safety and quality initiatives to enhance resident outcomes

Qualifications

• Active LPN or RN license (applicable to role you are applying for)
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Experience in long-term care, rehab or skilled nursing preferred but new grads with strong clinical skills are welcome
• Strong communication skills and ability to work collaboratively in a team
• Ability to pass background check and meet facility onboarding requirements

Education

• Graduate of an accredited practical nursing or registered nursing program (LPN diploma or RN diploma/associate/BSN as applicable)

Knowledge & Skills

• Solid knowledge of nursing fundamentals and clinical assessment
• Medication administration and documentation accuracy
• Familiarity with electronic health record systems
• Skilled at managing time, prioritizing tasks and multitasking in a busy environment
• Compassionate resident-centered care and strong interpersonal skills
