Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents as a licensed practical nurse
• Administer medications and treatments safely and according to orders
• Perform thorough resident assessments and document changes in condition promptly
• Collaborate with the interdisciplinary care team to create and follow individualized care plans
• Maintain accurate, timely documentation in resident records and electronic systems
• Implement infection control practices and promote a safe environment for residents
• Support residents' dignity and independence while delivering direct patient care

Qualifications

• Active LPN license (must meet facility and state requirements)
• Current basic life support (BLS) certification per facility policy or willingness to obtain
• Previous LPN nursing experience in a long-term care or similar setting preferred
• Comfortable administering medications, wound care basics, and conducting resident assessments
• Ability to provide your own reliable transportation to the facility (facility notes that ride-sharing alone may not be reliable)

Knowledge & Skills

• Solid knowledge of medication administration and documentation best practices
• Skilled in resident assessment, care planning, and recognizing changes in condition
• Familiar with infection control standards and safe patient handling
• Comfortable using electronic medical records and charting tools
• Strong communication and teamwork skills with residents, families, and clinical staff
