Roles & Responsibilities
- Provide compassionate, high-quality nursing care to residents as a licensed practical nurse
- Administer medications and treatments safely and according to orders
- Perform thorough resident assessments and document changes in condition promptly
- Collaborate with the interdisciplinary care team to create and follow individualized care plans
- Maintain accurate, timely documentation in resident records and electronic systems
- Implement infection control practices and promote a safe environment for residents
- Support residents' dignity and independence while delivering direct patient care
Qualifications
- Active LPN license (must meet facility and state requirements)
- Current basic life support (BLS) certification per facility policy or willingness to obtain
- Previous LPN nursing experience in a long-term care or similar setting preferred
- Comfortable administering medications, wound care basics, and conducting resident assessments
- Ability to provide your own reliable transportation to the facility (facility notes that ride-sharing alone may not be reliable)
Knowledge & Skills
- Solid knowledge of medication administration and documentation best practices
- Skilled in resident assessment, care planning, and recognizing changes in condition
- Familiar with infection control standards and safe patient handling
- Comfortable using electronic medical records and charting tools
- Strong communication and teamwork skills with residents, families, and clinical staff
