Roles & Responsibilities
- Provide compassionate direct patient care and support residents with activities of daily living
- Administer medications and treatments accurately and document per facility policy
- Assess and monitor patient condition, reporting changes to the interdisciplinary team
- Create clear, timely charting in the electronic health record and maintain clinical documentation
- Collaborate with nurses, therapists and facility staff to implement individualized care plans
- Promote a safe, clean and comfortable environment and follow infection control standards
- Support admissions, discharges and transfers as needed while maintaining patient dignity
Qualifications
- Current LPN license or Practical Nurse credential; Maine licensure or eligibility preferred
- Current BLS/CPR certification
- Ability to pass background check and meet facility immunization requirements
- Reliable transportation required; facility notes that rideshare services may be unreliable in the area
- Strong communication skills and a team-first attitude
- Flexibility to work assigned evening schedule for the duration of the assignment
Education
- Practical Nursing diploma or equivalent required
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and monitoring clinical changes
- Familiarity with standard infection control procedures
- Comfortable using electronic health records for charting
- Time management and prioritization in a fast-paced care setting
- Professional communication with patients, families and the care team
