Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day); No weekend rotation; 30 hours per week

Roles & Responsibilities
- Assess patients' functional abilities and develop individualized treatment plans
- Implement therapeutic interventions to improve mobility, strength, and safety
- Document patient progress and update care plans in the medical record promptly
- Collaborate with nursing, therapy aides, and interdisciplinary teams to optimize outcomes
- Educate patients and families on exercises, home programs, and fall prevention strategies
- Participate in discharge planning to ensure safe transitions and appropriate follow-up
- Monitor patient response to treatment and adjust interventions as needed
- Mentor and guide support staff to maintain high-quality care standards
Qualifications
- Current Physical Therapist license in Wisconsin (or eligible to obtain)
- Doctor of Physical Therapy (DPT) preferred or Bachelor's/Master's in Physical Therapy accepted
- BLS/CPR certification required (or willing to obtain upon hire)
- Experience in geriatric, outpatient, or rehabilitation settings preferred
- Strong clinical assessment skills, documentation accuracy, and patient-centered approach
- Excellent communication skills and ability to work effectively within a team
Education
- Doctor of Physical Therapy (DPT) preferred; Bachelor's or Master's in Physical Therapy acceptable
- State licensure required
Knowledge & Skills
- Skilled in musculoskeletal and neurological therapeutic interventions
- Proficient with patient assessment, goal setting, and outcome measurement
- Familiarity with electronic medical records and timely clinical documentation
- Strong patient education, coaching, and motivational skills
- Knowledge of safe transfer techniques, fall prevention, and mobility aids
- Ability to collaborate across disciplines and contribute to care planning
