Roles & Responsibilities

• Assess patients’ functional status and needs using evidence-based evaluation techniques
• Develop individualized treatment plans that target mobility, strength, pain management and return-to-function goals
• Deliver skilled therapeutic interventions including gait training, therapeutic exercise and manual therapy
• Collaborate with nurses, physicians and rehab team members to coordinate safe, effective patient care
• Document patient progress thoroughly and timely in the facility EMR
• Educate patients and families on home programs and strategies to maintain gains after discharge
• Participate in interdisciplinary team meetings and contribute to quality improvement initiatives

Qualifications

• Valid Physical Therapist license in Delaware or eligible to obtain state licensure
• Graduation from an accredited DPT program preferred (or equivalent physical therapy degree)
• Current Basic Life Support (BLS) certification (or willingness to obtain prior to start)
• Comfort working with a diverse patient population across the care continuum
• Strong communication skills and the ability to collaborate effectively with a multidisciplinary team
• Prior experience in rehabilitation or outpatient settings is a plus but motivated new grads are welcome

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent physical therapy degree from an accredited program

Knowledge & Skills

• Skilled in patient assessment, treatment planning and goal setting
• Proficient with gait training, therapeutic exercise progression and manual techniques
• Knowledge of safe mobility and transfer techniques, assistive devices and fall prevention
• Familiarity with documentation standards and electronic medical records
• Strong clinical reasoning, time management and patient-family education skills
