Candidate Pay Rate: $40 per hour

Roles & Responsibilities
- Evaluate patients and perform thorough assessments to build individualized treatment plans
- Develop and implement evidence-based therapy programs focused on function and mobility
- Provide hands-on therapy, therapeutic exercise, and adaptive strategies to maximize independence
- Collaborate with nursing and interdisciplinary team members to coordinate care and progress
- Document patient progress clearly and timely in the medical record to ensure continuity
- Educate patients and families on home programs, safety, and strategies for long-term success
- Participate in meetings and contribute to a positive, patient-centered facility culture
Qualifications
- Current Physical Therapist license (eligible for New Hampshire licensure or already licensed)
- Degree in Physical Therapy (DPT or Master's) — see education section for details
- Basic Life Support (BLS) certification required
- Experience working in a Skilled Nursing Facility (SNF) or geriatric population preferred
- Strong clinical assessment, documentation, and communication skills
- Ability to pass background checks and meet facility onboarding requirements
Education
- Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy (or equivalent)
Knowledge & Skills
- Skilled in assessment and treatment of mobility, balance, and functional impairments
- Knowledge of SNF rehab processes and regulatory/documentation requirements
- Proficient with therapeutic modalities, gait training, transfers, and adaptive equipment
- Ability to develop measurable goals and demonstrate clinical reasoning
- Excellent interpersonal skills for coaching patients and collaborating with care teams
