Roles & Responsibilities

• Evaluate patients and perform thorough assessments to build individualized treatment plans
• Develop and implement evidence-based therapy programs focused on function and mobility
• Provide hands-on therapy, therapeutic exercise, and adaptive strategies to maximize independence
• Collaborate with nursing and interdisciplinary team members to coordinate care and progress
• Document patient progress clearly and timely in the medical record to ensure continuity
• Educate patients and families on home programs, safety, and strategies for long-term success
• Participate in meetings and contribute to a positive, patient-centered facility culture

Qualifications

• Current Physical Therapist license (eligible for New Hampshire licensure or already licensed)
• Degree in Physical Therapy (DPT or Master's) — see education section for details
• Basic Life Support (BLS) certification required
• Experience working in a Skilled Nursing Facility (SNF) or geriatric population preferred
• Strong clinical assessment, documentation, and communication skills
• Ability to pass background checks and meet facility onboarding requirements

Education

• Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy (or equivalent)

Knowledge & Skills

• Skilled in assessment and treatment of mobility, balance, and functional impairments
• Knowledge of SNF rehab processes and regulatory/documentation requirements
• Proficient with therapeutic modalities, gait training, transfers, and adaptive equipment
• Ability to develop measurable goals and demonstrate clinical reasoning
• Excellent interpersonal skills for coaching patients and collaborating with care teams
