Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (day), 35 hours/week, No weekend rotation
Hours per Week: 40
Candidate Pay Rate: $38 per hour

Roles & Responsibilities
- Conduct comprehensive occupational therapy evaluations to identify patient needs and goals
- Develop individualized, evidence-based treatment plans focused on functional outcomes
- Provide skilled therapeutic interventions to improve independence with activities of daily living
- Monitor and document patient progress in the medical record with clear, timely notes
- Collaborate with an interdisciplinary team to coordinate care and support transitions
- Educate patients and families on strategies, adaptive equipment and home programs
- Participate in discharge planning and community referral to sustain gains made in therapy
- Contribute to quality improvement initiatives and maintain compliance with facility standards
Qualifications
- Current Illinois Occupational Therapist license (required)
- NBCOT certification preferred or active credentialing in progress
- Experience in outpatient, skilled nursing, or rehabilitation settings preferred
- Strong clinical reasoning with ability to create functional, patient-centered plans
- Excellent communication and collaboration skills to work within a multidisciplinary team
- Clear background check and ability to meet facility onboarding requirements
Education
- Graduate degree in Occupational Therapy (Master's or Doctorate) or equivalent entry-level degree
Knowledge & Skills
- Skilled in ADL retraining, adaptive equipment recommendations and therapeutic activities
- Proficient with assessment tools and writing measurable treatment goals
- Comfortable documenting in electronic health records and maintaining compliant charting
- Knowledge of safe patient handling, transfers and fall prevention strategies
- Ability to educate patients and caregivers in a clear, encouraging manner
- Time management and caseload prioritization to deliver high-quality care
