Roles & Responsibilities
- Provide compassionate, hands-on care for residents, assisting with activities of daily living
- Perform routine clinical tasks such as taking vital signs, bathing, dressing, and feeding with dignity
- Document resident status and care accurately and promptly to support continuity
- Communicate effectively with nurses, therapists, residents and families to coordinate great care
- Observe and report changes in resident condition to nursing staff to ensure safety
- Support a positive, person-centered environment that respects resident preferences and independence
Qualifications
- Current LNA or T-LNA certification (VT or NH as required for this assignment)
- Active state nursing assistant licensure appropriate for assignment
- CPR/BLS certification (or willingness to obtain upon hire)
- Ability to pass background check and drug screening per facility policy
- Previous experience in long-term care, rehab or similar settings preferred but new grads with strong skills are welcome
- Strong communication, empathy, and teamwork skills
Education
- High school diploma or equivalent preferred
- Completion of an approved LNA/T-LNA program and current certification
Knowledge & Skills
- Skilled in assisting with ADLs and promoting resident mobility and independence
- Ability to take and record accurate vital signs and basic clinical observations
- Knowledge of infection control practices and resident safety protocols
- Comfortable with clear, timely documentation and basic charting
- Strong interpersonal skills for working with residents, families and the care team
- Time management and prioritization to deliver dependable care during shifts
