Roles & Responsibilities

• Provide compassionate hands-on care to residents and patients, focusing on dignity and comfort
• Administer medications and treatments accurately and document care thoroughly
• Perform thorough patient assessments and communicate changes to the interdisciplinary team
• Collaborate with nursing staff and other departments to create and follow individualized care plans
• Support admissions, transfers, and discharges with clear communication and professional coordination
• Maintain accurate, timely clinical documentation and follow facility protocols for safety and compliance

Qualifications

• Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) credential — active and in good standing
• Basic Life Support (BLS/CPR) certification — current or ability to obtain promptly
• Comfortable working in long day shifts (6:45 AM - 7:15 PM) and adapting to unit needs
• Strong communication skills and a collaborative attitude with patients, families, and teammates
• Prior experience in long-term care, skilled nursing, or similar settings is a plus

Education

• Graduate of an accredited LPN or RN nursing program
• Completion of state licensure requirements for LPN or RN

Knowledge & Skills

• Medication administration and accurate charting
• Patient assessment, vital signs, and clinical observation
• Care planning and implementation with attention to patient safety
• Infection control practices and adherence to facility policies
• Ability to prioritize tasks and manage time during busy shifts
