Roles & Responsibilities

• Assess patients’ mobility, strength, balance, and functional independence to create targeted treatment plans
• Deliver evidence-based therapeutic interventions across orthopedic, neurological, stroke, balance, and chronic pain diagnoses
• Monitor patient progress and adapt care plans to maximize return to function and community participation
• Collaborate with nursing, OT, SLP, and physicians to coordinate holistic, patient-centered care
• Educate patients and families on home programs, fall prevention, and safe return-to-activity strategies
• Document evaluations and progress clearly and timely to ensure continuity of care
• Travel between multiple points of care within the service area to meet patients where they are

Qualifications

• Current unencumbered Oregon Physical Therapy license (required)
• Healthcare Provider level BLS (required)
• Comfortable traveling to multiple points of care as part of your daily routine
• Experience or interest in orthopedic, neurological, stroke, balance/dizziness, and chronic pain rehabilitation
• Strong communication skills and a collaborative approach to patient and team interactions

Knowledge & Skills

• Skilled in patient evaluation and treatment planning focused on mobility and functional independence
• Proficient in mobility assessment and designing individualized therapeutic programs
• Experience with orthopedic rehab techniques and post-injury recovery plans
• Knowledge of neurological and stroke rehab principles to optimize neuroplasticity and function
• Competence in balance and dizziness rehabilitation and fall prevention strategies
• Ability to manage chronic pain with movement-based approaches and patient education
• Comfortable working within an interdisciplinary rehabilitation team
