Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Assess patients’ mobility, strength, balance, and functional independence to create targeted treatment plans
- Deliver evidence-based therapeutic interventions across orthopedic, neurological, stroke, balance, and chronic pain diagnoses
- Monitor patient progress and adapt care plans to maximize return to function and community participation
- Collaborate with nursing, OT, SLP, and physicians to coordinate holistic, patient-centered care
- Educate patients and families on home programs, fall prevention, and safe return-to-activity strategies
- Document evaluations and progress clearly and timely to ensure continuity of care
- Travel between multiple points of care within the service area to meet patients where they are
Qualifications
- Current unencumbered Oregon Physical Therapy license (required)
- Healthcare Provider level BLS (required)
- Comfortable traveling to multiple points of care as part of your daily routine
- Experience or interest in orthopedic, neurological, stroke, balance/dizziness, and chronic pain rehabilitation
- Strong communication skills and a collaborative approach to patient and team interactions
Knowledge & Skills
- Skilled in patient evaluation and treatment planning focused on mobility and functional independence
- Proficient in mobility assessment and designing individualized therapeutic programs
- Experience with orthopedic rehab techniques and post-injury recovery plans
- Knowledge of neurological and stroke rehab principles to optimize neuroplasticity and function
- Competence in balance and dizziness rehabilitation and fall prevention strategies
- Ability to manage chronic pain with movement-based approaches and patient education
- Comfortable working within an interdisciplinary rehabilitation team
