Roles & Responsibilities
- Provide compassionate, evidence-based occupational therapy interventions under the direction of the OTR to help residents regain independence
- Implement individualized treatment plans and adapt activities to meet each resident’s functional goals
- Collaborate with nursing, rehab, and care team members to coordinate care and ensure consistent progress
- Document accurate, timely clinical notes and progress reports in the medical record
- Educate residents and families on home programs, adaptive equipment, and safety strategies to support long-term outcomes
- Monitor and report changes in resident status and participate in care plan meetings to optimize results
Qualifications
- Current COTA credential (Certified Occupational Therapy Assistant)
- Texas state licensure or eligibility to obtain Texas COTA licensure
- Current CPR/BLS certification (or willing to obtain prior to start)
- Experience in skilled nursing, long-term care, or rehab settings preferred
- Strong communication skills and ability to work collaboratively with interdisciplinary teams
- Ability to pass background and health screenings per facility policy
Education
- Associate degree from an accredited Occupational Therapy Assistant program or equivalent completion of an approved OTA curriculum
Knowledge & Skills
- Skilled in therapeutic activity design to improve ADLs and functional mobility
- Proficient with clinical documentation and outcome reporting
- Knowledge of adaptive equipment, splinting basics, and safe patient handling
- Ability to assess functional status and translate assessments into measurable goals
- Comfortable coaching family members and caregivers on carry-over strategies
