Roles & Responsibilities

• Communicate promptly and clearly with physicians, families, and the care team about resident condition changes
• Complete resident assessments thoroughly and on time to ensure personalized care plans
• Administer medications accurately following the 5 Rights and facility protocols
• Perform wound care and monitor healing, adjusting care as needed
• Assist CNAs and lead the floor when clinical direction is required
• Monitor residents for subtle changes and intervene or escalate appropriately
• Maintain accurate, timely charting and documentation for resident safety and compliance
• Report any allegations of abuse, neglect, or misappropriation to leadership immediately
• Support other duties as requested to ensure high-quality resident care

Qualifications

• Current Wisconsin Registered Nurse (RN) license required
• Associate degree in nursing (two-year degree) required; BSN preferred
• Current CPR/BLS certification required
• 1+ year(s) of related long-term care or nursing experience preferred
• Strong attention to detail and accuracy in clinical tasks and documentation
• Excellent communication and care coordination skills; able to lead on the floor

Education

• Associate Degree in Nursing (required)
• Bachelor of Science in Nursing (preferred)

Knowledge & Skills

• Medication administration and safe practices
• Wound care assessment and treatment
• Resident assessment and care planning
• Thorough charting and documentation skills
• Care coordination and clear communication with interdisciplinary teams
• Ability to supervise and mentor CNAs and other staff
• Knowledge of long-term care regulations and reporting requirements
