Contract Type: Travel Contract
Shift Type: 2:00 PM - 10:00 PM, Full Time PM shift, includes every other weekend (specific weekdays vary by facility schedule)
Hours per Week: 36
Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Communicate promptly and clearly with physicians, families, and the care team about resident condition changes
- Complete resident assessments thoroughly and on time to ensure personalized care plans
- Administer medications accurately following the 5 Rights and facility protocols
- Perform wound care and monitor healing, adjusting care as needed
- Assist CNAs and lead the floor when clinical direction is required
- Monitor residents for subtle changes and intervene or escalate appropriately
- Maintain accurate, timely charting and documentation for resident safety and compliance
- Report any allegations of abuse, neglect, or misappropriation to leadership immediately
- Support other duties as requested to ensure high-quality resident care
Qualifications
- Current Wisconsin Registered Nurse (RN) license required
- Associate degree in nursing (two-year degree) required; BSN preferred
- Current CPR/BLS certification required
- 1+ year(s) of related long-term care or nursing experience preferred
- Strong attention to detail and accuracy in clinical tasks and documentation
- Excellent communication and care coordination skills; able to lead on the floor
Education
- Associate Degree in Nursing (required)
- Bachelor of Science in Nursing (preferred)
Knowledge & Skills
- Medication administration and safe practices
- Wound care assessment and treatment
- Resident assessment and care planning
- Thorough charting and documentation skills
- Care coordination and clear communication with interdisciplinary teams
- Ability to supervise and mentor CNAs and other staff
- Knowledge of long-term care regulations and reporting requirements
