Roles & Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs to restore function and minimize disability
• Develop and implement individualized treatment plans with clear goals and measurable outcomes
• Perform initial admission and evaluation visits and complete timely OASIS reporting to notify the Clinical Team Manager
• Document observations, clinical findings, skilled interventions, goals and discharge plans accurately and professionally
• Supervise Physical Therapy Assistants and support a high standard of patient care
• Coordinate care and communicate with the clinical team, patients and families to promote best outcomes
• Travel within the assigned territory to provide in-home therapy services as needed

Qualifications

• Minimum one year of Home Health experience (required)
• Experience with OASIS documentation and timely reporting
• Proven ability to perform initial admission/evaluation visits and develop treatment plans
• Experience supervising Physical Therapy Assistants or working closely with rehab support staff
• Strong clinical documentation skills and attention to detail
• Comfortable traveling within the assigned territory to visit patients in their homes

Knowledge & Skills

• Skilled in patient evaluation and treatment planning for home health populations
• Proficient in thorough documentation and clinical recordkeeping
• Knowledge of home health best practices, safety assessment, and discharge planning
• Strong communication, time management and care coordination skills
• Ability to adapt treatment approaches to diverse patient needs and home environments
