Roles & Responsibilities

• Provide compassionate direct patient care including assistance with activities of daily living
• Administer medications and treatments safely according to facility policy and scope of practice
• Monitor and document patient vital signs, status changes, and care interventions accurately
• Collaborate with RNs, physicians, and the care team to coordinate excellent patient-centered care
• Educate patients and families about care plans, medications, and discharge instructions
• Maintain infection control standards and follow facility safety protocols
• Prioritize tasks and manage time effectively during busy 12.5-hour shifts
• Support a positive, team-first culture by communicating clearly and respectfully

Qualifications

• Current LPN license in New Hampshire (or compact/eligible license as required)
• BLS/CPR certification required; additional certifications welcomed
• Minimum relevant clinical experience preferred (long-term care, med-surg, or acute care experience a plus)
• Ability to pass background check, drug screening, and meet facility immunization requirements
• Strong communication skills and a patient-centered attitude
• Willingness to work scheduled weekends and participate in rotating weekend coverage

Education

• Graduate of an accredited Practical Nursing program
• Current LPN license and required state certifications

Knowledge & Skills

• Solid knowledge of nursing fundamentals and medication administration
• Proficient documentation in electronic health records and accurate charting
• Skilled in patient assessment, monitoring, and recognizing changes in condition
• Good time management and prioritization during extended shifts
• Team collaboration, clear communication, and a calm approach in high-pressure situations
