Roles & Responsibilities

• Provide skilled therapeutic interventions in a skilled nursing setting to improve mobility and independence
• Assist with gait training, transfers, and safe mobility techniques to reduce fall risk
• Collaborate with the interdisciplinary care team to develop and implement patient-centered treatment plans
• Document progress and outcomes accurately and timely in the patient record
• Educate residents and family members on exercise programs and safe mobility strategies
• Monitor patient response to treatment and communicate changes to the supervising therapist

Qualifications

• Current and unrestricted Physical Therapist Assistant license in California
• Completion of an accredited PTA program
• Basic Life Support (BLS) certification or willingness to obtain upon hire
• Experience in skilled nursing or long-term care settings preferred
• Strong communication and teamwork skills with a compassionate bedside manner
• Ability to follow care plans and document concisely and accurately

Education

• Associate degree from an accredited Physical Therapist Assistant program

Knowledge & Skills

• Skilled in therapeutic exercise, manual techniques, and functional mobility training
• Knowledge of safe transfer techniques and use of assistive devices
• Proficient with clinical documentation and outcome measurement
• Ability to adapt interventions for older adults and those with complex medical needs
• Good problem-solving skills and the ability to collaborate with nursing and rehab team members
