Roles & Responsibilities
- Assess residents' functional mobility and document findings clearly
- Implement therapeutic interventions and assist with restorative programs
- Collaborate with RNs, PTs, OT and interdisciplinary team to deliver person-centered care
- Monitor progress and modify treatment plans to meet goals
- Provide safe patient transfer and fall-prevention techniques
- Educate residents and families on exercise programs and safe mobility strategies
- Maintain accurate, timely clinical documentation in the medical record
Qualifications
- Active Physical Therapist Assistant license in California (required)
- Graduate of an accredited PTA program
- Current BLS/CPR certification
- Experience in skilled nursing or geriatric rehabilitation preferred
- Strong communication skills and ability to work collaboratively with care teams
- Comfortable with electronic medical records and documentation
Education
- Associate degree from an accredited Physical Therapist Assistant program
- Completion of clinical PTA training and eligibility for CA licensure

- Geriatric rehabilitation principles and restorative care
- Therapeutic exercise and mobility training
- Safe transfer and fall-prevention techniques
- Skilled documentation and care planning
- Ability to prioritize tasks in a busy skilled nursing environment
- Patient- and family-centered communication skills
