Roles & Responsibilities
- Provide thoughtful therapeutic interventions to skilled nursing residents and help them regain function
- Assist in developing and implementing individualized treatment plans under a supervising PT
- Perform functional assessments, gait training, therapeutic exercise and modalities as appropriate
- Document patient progress clearly and on time to support continuity of care
- Collaborate with nurses, social workers and family members to ensure a patient-centered plan
- Support safe transfers, mobility and fall-prevention strategies for residents
- Educate residents and caregivers on home programs and adaptive strategies
- Maintain equipment and ensure a clean, safe treatment environment
Qualifications
- Licensed Physical Therapist Assistant (PTA) in California or eligible for state licensure
- Graduate of an accredited PTA program (associate degree or equivalent)
- Current CPR/BLS certification
- Experience in skilled nursing or geriatric care preferred
- Ability to pass background checks and required health screenings
- Strong communication skills and a team-oriented mindset
Education
- Associate Degree from an accredited Physical Therapist Assistant program
Knowledge & Skills
- Skilled in therapeutic exercise, gait training, balance and mobility interventions
- Knowledge of documentation standards in long-term and skilled nursing settings
- Ability to adapt treatment plans for complex, medically-fragile residents
- Familiarity with assistive devices and safe transfer techniques
- Good clinical judgement, time management, and patient education skills
- Comfort collaborating with interdisciplinary teams
