Roles & Responsibilities
- Deliver skilled physical therapy interventions under the direction of a supervising PT to improve patient mobility and function
- Perform therapeutic exercises, gait training, transfers, and safe patient handling with compassion and clinical skill
- Document patient status, progress, and treatment plans clearly and timely in the facility EMR
- Collaborate with nurses, therapists, and care staff to craft and implement individualized care plans
- Educate patients and families on home programs, fall prevention, and safe mobility strategies
- Maintain equipment and ensure a safe, clean treatment environment
- Participate in team meetings, discharge planning, and continuity of care to maximize patient outcomes
Qualifications
- Licensed Physical Therapist Assistant (PTA) in California (or eligible for California licensure)
- Graduate of an accredited PTA program
- Current CPR/BLS certification
- Experience in skilled nursing or post-acute care preferred
- Strong interpersonal and communication skills; comfortable working with an interdisciplinary care team
- Ability to perform physical tasks required for patient transfers and mobility training
Education
- Associate degree or completion of an accredited Physical Therapist Assistant (PTA) program
Knowledge & Skills
- Solid understanding of therapeutic modalities, biomechanics, and safe patient handling
- Skilled in gait training, balance training, transfers, and ADL-focused interventions
- Familiarity with electronic medical records (EMR) and timely clinical documentation
- Ability to assess patient progress and modify treatment under PT guidance
- Strong observational skills and clinical reasoning
- Professional bedside manner and patient-centered communication
