Roles & Responsibilities
- Assess patients in skilled nursing to determine mobility, strength, balance, and functional needs
- Develop and implement individualized treatment plans that promote independence
- Provide hands-on therapy, gait training, and therapeutic exercises with compassion
- Document progress clearly and timely in the facility’s clinical record system
- Collaborate with nurses, occupational therapists, speech therapists and physicians to deliver coordinated care
- Educate patients and families on home programs, safety strategies, and fall prevention
- Participate in discharge planning to optimize patient transitions and outcomes
- Maintain compliance with facility policies, regulatory requirements, and best practices
Qualifications
- Current, unrestricted California Physical Therapy license or ability to obtain licensure quickly
- Graduation from an accredited physical therapy program (DPT preferred or equivalent)
- Current Basic Life Support (BLS) / CPR certification
- Experience in skilled nursing or post-acute care preferred
- Strong clinical reasoning, documentation skills, and a patient-centered approach
- Ability to pass background checks and meet facility onboarding requirements
Education
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
- Completion of required clinical internships and state licensure
Knowledge & Skills
- Proficiency in gait training, balance retraining, transfers and therapeutic exercise
- Comfortable with patient handling techniques and adaptive equipment
- Experience with documenting in electronic medical records and managing caseloads
- Knowledge of skilled nursing regulations, Medicare documentation expectations, and rehab pathways
- Excellent communication skills for patient education and interdisciplinary collaboration
- Ability to prioritize, stay organized, and adapt to changing clinical needs
