Roles & Responsibilities

• Assess patients in skilled nursing to determine mobility, strength, balance, and functional needs
• Develop and implement individualized treatment plans that promote independence
• Provide hands-on therapy, gait training, and therapeutic exercises with compassion
• Document progress clearly and timely in the facility’s clinical record system
• Collaborate with nurses, occupational therapists, speech therapists and physicians to deliver coordinated care
• Educate patients and families on home programs, safety strategies, and fall prevention
• Participate in discharge planning to optimize patient transitions and outcomes
• Maintain compliance with facility policies, regulatory requirements, and best practices

Qualifications

• Current, unrestricted California Physical Therapy license or ability to obtain licensure quickly
• Graduation from an accredited physical therapy program (DPT preferred or equivalent)
• Current Basic Life Support (BLS) / CPR certification
• Experience in skilled nursing or post-acute care preferred
• Strong clinical reasoning, documentation skills, and a patient-centered approach
• Ability to pass background checks and meet facility onboarding requirements

Education

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
• Completion of required clinical internships and state licensure

Knowledge & Skills

• Proficiency in gait training, balance retraining, transfers and therapeutic exercise
• Comfortable with patient handling techniques and adaptive equipment
• Experience with documenting in electronic medical records and managing caseloads
• Knowledge of skilled nursing regulations, Medicare documentation expectations, and rehab pathways
• Excellent communication skills for patient education and interdisciplinary collaboration
• Ability to prioritize, stay organized, and adapt to changing clinical needs
